Psychiatric Nurse Practitioner

M-F 9-5

On a day-to-day basis, the Psych NP conducts psychiatric evaluations, diagnoses mental health conditions, manages psychiatric medications, provides supportive counseling, responds to crisis situations, and coordinates referrals for ongoing behavioral health or substance abuse treatment. The provider typically sees about 4–5 patients per day, occasionally up to 8 including walk-ins or urgent consults, allowing for more relationship-based and thorough patient care compared to high-volume inpatient psychiatric settings. The patient population includes all ages struggling with depression, anxiety, PTSD, mood disorders, trauma, substance abuse issues, housing instability, and other psychosocial challenges. Many patients are identified through primary care visits and referred internally when behavioral health concerns arise.

The role is highly collaborative and works closely with psychiatrists, primary care providers, therapists, social workers, care coordinators, shelter staff, and outside mental health programs to ensure patients receive comprehensive support and continuity of care. At a broader level, the position is focused on stabilizing patients, improving access to psychiatric care, reducing psychiatric emergencies and hospitalizations, and integrating mental health treatment into overall medical care.

This is a lower-volume outpatient clinic environment with a Monday through Friday schedule, offering a more balanced and preventative approach to psychiatric care rather than the high-acuity pace of inpatient psych units or psychiatric emergency departments.

DUTIES:

? Performs psychiatric evaluations and assessments for patients to ascertain their diagnoses and establish treatment plans

? Collaborate with TFH Psychiatrist pertaining to documentation and clinical decisions

? Provides SBIRT encounters as a part of the Substance Abuse Expansion and Integration Program

? Provides medication management including education/review of side effects and treatment options with patients so they may give informed consent

? Provides individual psychotherapy

? Provides crisis intervention and/or counseling to diffuse conflicts among patients

? Completes medical forms and documentation related to patient care including any necessary to facilitate transfer, and if indicated, referrals to outside programs or psychiatric clinics

? Makes and/or tracks all referrals to other mental health agencies and providers

? Consults with primary care providers to focus on psychiatric and substance abuse issues

? Works collaboratively with specialists for consultation and referral purposes

? Coordinates patient's psychiatric care with medical clinic and shelter staff, as well as outside providers

? Provide coverage for Psychiatrist at TFH regarding supervision of clinical staff when further guidance in patient care may be needed, in person or by telephone.

? Coordinates emergency responses, including a team to minimize patient's danger to self and others, direct discussion with EMS/FDNY regarding patient's condition, referral to psychiatric or medical emergency departments and follow-up while patient is in the emergency department or upon release

? Maintains ultimate responsibility for directing the course of the patient's psychiatric care and treatment plan within the scope of his/her expertise while collaborating with Psychiatrist at TFH

? Encourages health promotion, disease prevention, and health maintenance ? Provides patient advocacy, as appropriate for the site

? Participates in case-conference and case management activities Administrative Duties:

? Meets with TFH MH team members on a weekly basis, to discuss/implement recommendations and collaborate toward delivering quality psychiatric services to patients, which includes the Substance Abuse Expansion and Integration Program in addition to Article 31. Meetings are held at a central location, allowing clinicians to benefit from the experiences of their colleagues ?

? Collaborates with the Registration and Scheduling Departments to ensure appropriate patient scheduling and flow

? Collaborates with the HIM Department to ensure appropriate documentation procedures; discusses such issues with TFH MH clinical staff

? Observes, implements, and promotes the federal regulations as defined by HIPAA and Policies and Procedures as defined by The Company

? Makes suggestions to the Director of Behavioral Health and the Chief Medical Officer to improve patient care

QUALIFICATION: PMHNP, BC, DEA, 3+ year as a PMHNP with population exp with homeless - vulnerable, and underserved. medicaid billing number required. EMR Access includes the following actions/areas: Demographic, Registration, Appointment/Scheduling, Scanning, Triage, PMH/Vital, Orders, Immunizations/PPD, Progress Note, Reporting, Labs, Medial Summary, Telephone Encounters; Electronic Prescribing
